If you are interested in purchasing a puppy from us, please download the PDF application form below and follow these steps:

1. Make sure you have Adobe Acrobat Reader 8 or higher - if not, then follow this link to download the current version directly from Adobe.com. It's free, and just takes a couple of minutes.
2. Click on the Acrobat icon below. When the form opens, simply fill it in on your computer.
3. When you've completed the questions, go to "File > Save As" and save it, replacing "yourname_application.pdf" in the file name with (you guessed it!) your last name. THIS STEP IS IMPORTANT! Otherwise we may not be able to tell which application is yours, creating unnecessary delays in responding.
4. Using your normal email program, include the form as an attachment and send it to us at tails@tangowoollabradoodles.com

Please note that if you are a Mac user, your computer may use another program to open PDF documents by default. If this is the case, please make sure you save the form to your desktop and use Acrobat Reader to open and fill out the application.
